Applications for data access can be submitted by filling out this form and sending it to the UMC Utrecht by clicking on the button [Submit] at the end of the form.

Research support staff in the research departments/divisions (data manager, legal, privacy ed.) will review and consider applications and respond within 6 weeks. Applicants must provide full contact details of their institution. Please use your institute email addresses as private email addresses are not accepted. Incomplete applications will not be taken into consideration.
